- Completed Supervised Practice Program Application (available at www.depdpg.org).
- A letter of application that includes both short-term and long-term professional goals.
- The completed Declaration of Intent Form signed by the program director of the ADA-approved didactic program in dietetics where the applicant is currently enrolled or the Verification of Completion of Program Form for an applicant who has already graduated. This form is available at the applicant's school through the program director of the ADA-approved didactic program in dietetics.
- Two copies of official transcripts from all institutions of higher education previously attended.
- Graduate Record Examination scores taken within the last five years. Indicate the Field as number 0214 (Nutrition) and the College as number 3263 (Rush University) on the GRE application materials.
- Three letters of recommendation from professional persons qualified to give pertinent information as to the ability and professional qualifications of the applicant. It is requested that one letter be from a nutrition faculty member and one from a management faculty member. Recommendations must be written on standard ADA forms and must be in individually-sealed envelopes within the application packet. Sender signatures should be written across the envelope seal.
- Documentation of successful completion of a college course in basic statistics.
- Applicants who have obtained their education outside the United States and its territories must have their academic degree(s) validated as equivalent to the baccalaureate or master's degree conferred by a regionally accredited college or university in the United States. These applicants also must submit results of the TOEFL examination.

All applicants to dietetic internships must participate in computer matching. Applicants should request instructions and forms from the director of any ADA-approved didactic program in dietetics or from D&D Digital System. A $50 fee is charged for computer matching. Payment should be submitted along with your completed forms to:

Drug Testing
Rush University Medical Center requires that all prospective employees (including dietetic interns) undergo drug testing as a contingency for employment or enrollment.
Criminal Background Check
All dietetic interns will undergo a criminal background check, the purpose of which is to comply with legislation regarding employment in the health care field to assure patient safety.

- Copy of the Commission on Dietetic Registration card documenting status as a registered dietitian.
Applicants who have obtained their education outside the United States and its territories must have their academic degree(s) validated as equivalent to the baccalaureate or master's degree conferred by a regionally accredited college or university in the United States. These applicants also must submit results of TOEFL examination. The generally applied guideline for acceptance into the program is a "B" average for undergraduate achievement and GRE scores above the 50th percentile.
